The Hospice Care Team & Services

Hospice Care Services are individualized to the patient receiving care. After meeting with and evaluating the patient, our Hospice Care team will create a care plan that addresses the Physical, Spiritual, and Emotional care needs of the patient. The Fundamental Hospice Care team will regularly visit the patient in accordance with the care plan, but a member of our Hospice Care staff is always available for patients on-call 24-Hours a day at (747) 336-3195. The Fundamental Care Hospice Team consists of our Medical Directors, Registered Nurses (RN), Licensed Vocational Nurses (LVN), Hospice Aides (HA), Social Workers, Physical Therapy (PT), Occupational Therapy (OT), Speech Therapy (ST), Chaplains, Bereavements Counselors, and amazing volunteers.

HOSPICE CARE REPRESENTATIVE

Our Hospice Care Representative is the Community Liaison. They will meet with you and your family prior to the start of Hospice Care to discuss all of your care needs. The Fundamental Hospice Care Representative  will coordinate the Nursing, Social Work, and Spiritual Counseling assessments to create a personalized Hospice Care Plan and to answer all of your Hospice Care questions

HOSPICE CARE PHYSICIAN

Our Hospice Physician oversees the Nursing Team that is in charge of creating a specialized plan of care for each patient. The goal of this specialized care plan is to ensure comfort and quality of life for the patient through end-of-life care. The Hospice Physician works with all members of our Hospice Care Team, and available for home visits. 

NURSING

Our Hospice Nursing Team is comprised of Registered Nurses and Licensed Vocational Nurses who manage patient care, including care related to pain or symptoms of discomfort. Our nurses provide assistance and knowledge to patients and their families in regards to the patient’s condition. The Fundamental Care Nursing Team is responsible for adjusting the care plan in accordance with the patient’s needs.

SOCIAL WORKER

A Fundamental Care Social Worker will help to assist and advocate for patients and their families every step of the way. Our Social Worker provides resources ranging from additional care services (caregiving) to matters like advance directives, and end-of-life planning.

HOSPICE AIDE

Hospice Aides are members of the Hospice Care Team that focus on one-to-one patient assistance. If needed,  Hospice Aides assist patients in the Activities of Daily Living, ADLs. These activities include bathing assistance, dressing assistance, meal/feeding assistance, personal hygiene assistance, and other ADLs.

This is how we provide hospice care most often: in patients’ homes, long-term care facilities and nursing homes. We offer hospice care at home to provide patients with comfort and dignity in their own home. Our team of experts provides medical care, emotional support, and assistance with daily living activities.

When medically necessary, acute symptom management is provided at home or in another facility by hospice staff in shifts of up to 24 hours/day so the patient can avoid hospitalization. We offer compassionate end-of-life care services to patients and their families. Our team of experts provides care and support to manage pain and symptoms and provide emotional support during this difficult time.

If a patient’s needs cannot be managed at home, local contracted nursing home facilities provide hospice care around the clock until the patient can return home. We provide specialized medical care for people with serious illnesses. Our focus is on providing relief from symptoms, pain, and stress of the illness. Our goal is to improve quality of life for both the patient and the family.

Respite Care

Inpatient Care

Limited to up to five consecutive days, respite care provides a brief “respite” for the patient’s primary caregiver by admitting the home care patient to an institutional setting without meeting the “inpatient” pain and symptom management criteria.
