The hospice team is made up of healthcare professionals who work together to provide comprehensive care for the patient and their family. This may include a doctor, nurse, social worker, chaplain, and other specialists as needed.
Hospice Care eligibility is dependent on patient diagnosis. Hospice Care provides a care-plan that highlights the patient’s needs/wants with a focus on comfort care and quality of life for patients nearing their end of life.
Before hospice services can begin, we must receive a referral.
The referral process consists of assisting the patient and family in checking the patient's insurance for hospice service coverage. Hospice accepts Medicare, MediCal, and other private insurances. Once the Hospice representative updates the patient and their family of their insurance coverage the representative will visit the patient within 48 hours.
During the visit, the representative will go over information which includes the explanation of benefits (EOB) and the assessment and evaluation of the eligibility of the patient.
If the patient is deemed eligible to receive hospice services, the hospice team will collaborate with the patient / family to create the appropriate care plan for the patient.
Yes, hospice care can be provided to individuals who are still receiving treatment for their illness. In fact, hospice care can be provided alongside curative treatments to help manage symptoms and provide emotional support.
Yes, you can change your mind about hospice care at any time. Our team will work with you to understand your needs and preferences and make changes to the care plan as needed.
